Before moving in, our team will meet your loved one in their home to learn more about their everyday activities, their preferences, and to assess their health and personal care needs. Upon completion of the assessment, our health care services team will prepare a comprehensive personalized service plan and determine their Level of Care. 

At The Heritage at Meridian Gardens, we offer our specialized Snapshots program for residents with mild cognitive impairments who may be in the middle place between assisted living and memory care. Through wellness checks, facilitated small groups and one-on-one activities, and supplemental care, we promote mental and emotional wellness so your loved one can feel continuously engaged and supported within our community.

Whether you can’t get enough of our soft-serve ice cream or our loaded grilled cheese, eating with us is always a pleasure. If your loved one doesn’t care for the main entrees of the day, they are welcome to order something from our alternative menu so they can always find a meal to enjoy.
